Thumbnails not matching full size images

Over the past couple of weeks I have had several experiences where the thumbnail I click on brings up a completely different image.

Sometimes it is just one or two images off, other times the full size image that loads appears to come from a completely different file.

There does not appear to be any rhyme or reason to this issue except that is happening more frequently.

It's happening for me too. It probably occurs 90% of the time.
